Medication Therapy Management (MTM)
HAP’s Medication Therapy Management (MTM) program is a free service that can help find prescription errors and lower your costs. It will also help make sure you’re using the best therapies to treat your medical conditions.
How can I take advantage of MTM?
In 2017, if you use eight or more medications and spend over $3,919 on medications annually, you will be eligible for our Medication Therapy Management (MTM) Program.
- If you are eligible, HAP will mail you a letter explaining the program and the potential benefits.
- An MTM analyst will call you after the letter mailing to help answer any questions you may have on the MTM Program and to set up a telephone appointment with one of our specially trained MTM pharmacists.
HAP wants to make sure all eligible beneficiaries are given the opportunity to participate in the Medication Therapy Management program.
How does it work?
Once you are enrolled, a pharmacist will contact you by phone at a conveniently scheduled appointment time that you choose.
During your telephone appointment, an MTM pharmacist will conduct a Comprehensive Medication Review (CMR), in order to:
- Discuss your medications and answer any questions
- Identify any possible drug interactions
- Highlight potential ways to reduce your drug costs
- Determine if any medicines need to be adjusted for medication safety or to improve your health
After completing the CMR, the pharmacist will mail to you a cover letter sharing your MTM pharmacist’s name and contact information, an Action Plan listing all discussed medication-related concerns and any follow-up steps to be taken. You will also receive a Personal Medication List which includes your prescription and over-the-counter medications, how to take the drug, why you are taking the drug and who prescribed the drug. In addition to the initial comprehensive medication review, all patients who are eligible for the MTM Program will be contacted by letter and phone every three months to help answer any questions you may have about your medications and/or help to make sure you are taking your medications as prescribed by your doctor. This outreach is referred to as a Targeted Medication Review (TMR). CMS requires that all beneficiaries enrolled in MTM receive a TMR every three months.
HAP members who are in the MTM Program do NOT need to change their current physician or pharmacy.
MTM is not considered a Medicare benefit.
